This is the patient’s personal doctor. He / she maintains a leading role and is a key member of the hospice team. They are responsible for the supervision and approval of the patient’s care plan.
The hospice medical director works hand in hand with the primary physician to supervise and approve the patient’s care plan. He / she may also serve as a patient’s primary physician when necessary.
The hospice RN is the patient’s case manager. The hospice RN and LPN work alongside the patient’s doctor, suggesting solutions regarding pain and symptom management. They are responsible for the patient’s general physical comfort and coordination of the patient’s care with the interdisciplinary team.
The CNA is supervised by the RN and is responsible for assisting with the patient’s personal care. Homemakers provide help with personal needs which may include light housekeeping.
The hospice chaplain provides spiritual support to every patient and their family members requesting such. The hospice chaplain serves as the bridge between patient / family and their usual place of worship. A chaplain can also help to find a clergy of your faith. At Angel Hospice, our patients and families have access to our 24-hour prayer line.
The hospice social worker provides overall support forpatients and their family. Social workers can assist with financial planning, advance directives as well as counseling and emotional support.
The hospice volunteer plays a very special role in the plan of care. They are trained to provide a special kind of caring, companionship and respite for our patients and their families. They may also provide emotional and bereavement support, office help and assistance in many other ways.
Medical equipment and medication directly related to your hospice diagnosis will be provided. This is normally provided through a contracted vendor.
Additional support and comprehensive bereavement services are available for the families of hospice patients, as well as community members, following the loss of a loved one. This may include home visits by staff and / or volunteers, mailings and phone calls. Bereavement support is available for up to 13 months after the loss.
A music therapy session through Angel Hospice is a goal-directed therapy with a person-centered approach. The music therapist uses music to address and improve each patient’s quality of life.
These therapists will be utilized as needed to improve our patients’ quality of life.
